Most transwomen are all too familiar with the trademark effects of taking estrogen – softer skin, fat redistribution, boobs, etc, etc. But there are loads of other MTF HRT effects that can kick in to a greater or lesser extent. I’ve been doing my research.
So you’re a transwoman and you’re about to start your HRT journey. Congratulations, girl! Get yourself ready for a bigger booty and a trimmer tum, and prepare to say goodbye to your body hair and possibly even your erections.
These are all common effects of MTF HRT, and pretty much all transwomen will experience all of them, to one degree or another. Here’s a timeline of what’s what:
As well as these effects, you can add in the fact that you’ll likely feel a lot happier (handy for those of us with depression), your face will become more rounded and feminine, and you’ll become a lot more emotional – get the Kleenex ready.
These are all pretty well-known facts about starting estrogen and testosterone blockers. And, seeing as I’m about to start my own HRT next month, I thought I’d see if there was anything else that may be in store but doesn’t get talked about quite so much.
And the internet is full of anecdotal evidence from transwomen reporting other symptoms of MTF HRT. Here’s my top ten, in no particular order.
1. Orgasms
Men’s and women’s orgasms are completely different. A male orgasm is pretty much centred around the genitals and stomach area, whereas a female orgasm can affect the whole body, last far longer and be far stronger. I once read that they’re ten times stronger than male orgasms. So while HRT might cause shrinking and reduced erections, on the flipside of the coin is the fact that transwomen can get female orgasms. And you don’t need an erection or an ejaculation to have one either.
2. Smell
I’ve read quite a lot of comments from transwomen who report a massively improved sense of smell. That’s really good from my point of view, because mine ain’t the greatest and never has been. Your body scent will also change as you start pumping out sweet female pheromones rather than the musky male ones. Be warned, though, apparently the smell of men really stands out to transwomen (and cis women) – not great on a hot summer’s day when they’re a bit sweaty. Especially if you’re not into guys.
3. Sexuality
Which brings us nicely on to sexuality. In most cases, your sexuality ain’t gonna change. If you like smooching girls before HRT, you’ll still like smooching them afterwards. But I’ve read a few accounts of transwomen becoming attracted to men after starting HRT. Probably not enough that they’d go off girls, but it might flick a bisexual/pansexual switch in some people.
4. Eyelashes
This is a new one on me, but apparently HRT can make your eyelashes thicker, longer and more feminine. A nice little beauty bonus there for trans girls who care about how their eyes look.
5. Extremities
It’s pretty well known already that taking estrogen causes huge reduction in muscle mass, making the body appear slimmer and more feminine. You’ll also lose a shed load of strength. What I didn’t realise until recently is that this can also cause your hands and feet to shrink – and I’ve even read accounts of people losing an inch or two in height.
6. Taste
Some transwomen report that their sense of taste changes – they might go off foods that they’d always eaten before and start to like those that they’d previously avoided. Cravings can be common. It’s not just taste in food that can change either. I’ve read a few accounts of people’s taste in terms of music and TV changing.
7. Chocolate
Most women love chocolate, but I’ve never really realised why – it’s just been a nice sweet treat to me up to now. But add a little estrogen to your system and chocolate becomes a drug, helping ease stress, combat fatigue and just make you feel awesome. Looking forward to that!
8. Nails
One of the bad things about HRT is that it can make your fingernails more fragile, which sucks because mine snap all the time as it is, especially in the winter. Might be time to start using my little bottle of almond oil on them to keep them supple.
9. Cold
Where I’m from, we have a word for people who feel the cold more than most: nesh. Despite the extra layers of fat, rumour has it that HRT can make transwomen feel the cold more than before. And when you look at how unbelievably thin most women’s jumpers are, that’s gonna be a pain in the arse. Time to order some thermals perhaps.
10. Spots
Starting MTF HRT is literally starting a second puberty (unless you transition in your teens). So you can expect all of the crap that you had to put up with the first time round, such as spots and mood swings. Deep joy. At least my voice won’t break this time – and I won’t start growing a shit beard.

Just finally, I’ve decided to take a short career break for a few weeks so that I can concentrate on myself (and do some work on my house) while I start MTF HRT. As the time draws nearer, I am just getting more and more happy, and more and more excited. The feelings of apprehension are still there, but they’re far less prominent than they once were. I can’t wait to get started!

So much of this matches my own experiences. Losing the man smell was a lovely and unexpected surprise I’d never really considered before. I have experienced reduction in foot size, height and it seems hands – some rings I used to wear now just fall off. Oh that cold though – any trans girl starting HRT prepare to experience feeling the cold as never before. As for a flatter belly, I could do with a bit more of that effect.

The blood vessels in my hands and arms shrank so much you can barely even see them (unless I’m too warm). They used to bulge out all the time! They also freeze fast in cold weather and are impossible to warm back up without external heat.

Am looking for therapists in my area to talk about my gender. And one that takes my health coverage insurance. I am heterosexual but absolutely love and get stimulation by wearing things girls would and painting my nails.
Recently started reaching out to long ago GF, cousin and some select female friends. EVERYONE of them have been so supportive. All of them have husbands and children. No association with Transgender issues. Brave step in sharing my undefinable feelings.
A huge step,was going to the public pool with my 9 year old son and his female 9 year old friend. I went with shaved body and painted toenails. My son, for now, thinks it is fine to appear how you feel you should. His friend said she liked the color. We were there for 4 hours. I work sunglasses so I could keep an eye out for reactions. The only one I noticed was two teenage girls. One noticed and elbowed her friend to take a look. They giggled a little.
The coolest thing was I was gaining confidence. That afternoon I decided to just sit on my front porch barefooted. Never met my neighbor across the street. I am grow cucumbers for pickling on my porch. I called out to her, “Do you guys like pickles?” She couldn’t quite hear me so she said she would come over to hear what I was saying. As she walked over, while carrying her infant, I thought Oh Shit. Then I got over it and stood my ground. As she walked up she saw my toes and said nothing. I offered her to sit down. She did and we started introducing ourselves. After about 5 minutes I called my toenails to here attention. She said she saw them and liked them. Score on for my fears diminishing. She went on to chat for about 4 hours. She even came in to my house with her two daughters and son along with infant. Her three older kids all said they liked the shade of my nails without anyone even pointing my toes out to them. My son was there too and he said yeah it is cool. The kids all ran off to play.
Back to conversating with her. She revealed a lot about her emotional issues with her life. Eventually she told me she was “fascinated” by me and resolve in girl looks. She even told me I could walk through her closet anytime.
Since then, we have been developing a friendship of support.
It was a weekend of success and I feel so,happy. I do get a some crap at work. I stand up for it and shoot down the snarky remarks. Now granted, all they see is me wearing headbands , tie dyed ankle socks and shaved legs. I retort comments privately with my beliefs. They seemed to kind of respect that.
Long way to go or not go.
Long winded, but I am possibly interested in SRS. Really interested in HRT.

I’ve been on hormones for a year now, I’ve lost more then an inch or two in height, my hands are significantly smaller as are my feet. I have gotten up to a c cup, I definitely am much colder then I used to be. I used to be one of those people who’d freeze everyone out cause I was hot even in the middle of winter. And now I can’t stand the AC for too long (at most 30 minutes). I’ve noticed smaller shoulders not as broad as mine used to be. And, one last thing… for me even though this is far out there… I have a very hard time talking in my normal male voice, and I never did any voice trainings my voice started becoming really hoarse all the time and I realized I had to raise my pitch cause talking with my normal male voice actually hurt and was making me very hoarse. So… that has been my experience.

I wanted to ask about relational timeline effects of Phytoestrogens vs Estrogens in MtF Transitioning. I have been on massive amounts of Phytoestrogens now for 8 months. These being mainly Fenugreek and Red Clover tops. I have also been supplementing Dong Qua, Black Cohosh, Saw Palmetto and MacaRoot. The problem being that to achieve the correct dosages (and I’ve done my extensive research), you have to take enough of the supplements to choke a horse… 2 times a day to reach the desired doses. I also eat very healthy (no fast foods or crap food from the convenience store), lots of protein and at least 32 oz of soy milk 2 times a day. I can leave the dosage info on a return post if that’s allowed. I started 3 months ago on taking Swanson’s products in hopes of reducing the amount of Phytoestrogens I am consuming. I take the multi glandular (Ovarian, Uterine, Kidney and Heart Glandular) twice a day and the ovarian glandular 3 times a day. with all of that said. I originally started on the fenugreek and the red clover as a change to taking pharmaceutical gluciose meds as I was diagnosed with diabetes after having a stroke 3 years ago. It seems to have done the trick as I am no longer a diabetic! The transition from MtF was always something I desired but I started budding after taking these supplements every day I have noticed all of the changes described in your posts including breast growth, mood changes, erection difficulty, orgasm effects and the weight distribution. All of this is not on massive scales but noticeable. I have grown into a healthy A cup and expect to grow more if I continue with my schedule. I am not sure if you can answer my question but was curious if anyone knew what to expect as far as changes of plant based estrogens as opposed to pharmaceuticay supplied estrogen? Thanks in advance!

I’m afraid I don’t have the answer to your question but I’d say be very careful with the phytos. I took them, thinking it would be an easy option, that they were far more mild than pharmaceutical E. But I soon got budding and then I got a DVT, which could have killed me. It’s essential you get your levels checked by an endo – or you could die. It’s that simple.

After taking the fist step in buying woman’s clothes, I found on a popular shopping site plant based estrogen supplements and began taking some. I have found that one in particular seems to work without any side effects. I have been taking it daily and have noticed a nicer butt, small breasts forming, getting colder, and my skin my gosh has gotten sooooo soft! I’m hoping this feminization will keep me looking young. a couple of women and a guy said i’m looking good so it must be working. I don’t plan to Op or do HRT but I do have a question after being on estrogen is it normal to want to buy women’s clothes?

Good luck with it all, Richelle. As ever, I’d warn you to get your hormone levels checked to avoid any risk of DVT or worse. As for your question, I’d say most trans women have a desire to wear clothes that fit in with their gender long before beginning HRT. It sounds like you’re questioning your gender. Most cis guys don’t have a girl name, take phytoestrogens or desire women’s clothes. I’d say just take it slowly and keep exploring – you’ll work it out for yourself one day. Andie x

Understood! I purposely stayed away from PM in lieu of less aggressive Phytoestrogens. After all it was for other medical reasons I did my research. I have just recently acquired medical insurance after not being qualified to have it. The US healthcare and insurance world has a lot to be desired of… I was unable to continue my blood glucose regiment for that reason and started researching other means to achieve my blood sugar goal. Its Ironic that all of the items I found worked for symptoms menopausal women have. Phytoestrogens help regulate the endocrine syste, including blood sugar levels. As a result of having a previous stroke, I have been on a 325mg aspirin regiment for the last 3 years to help thin the blood. I have a new set point for a regular 6 month doctor check in but I am currently in the best health I have ever been in including my teen age years. I appreciate your quick reply and I will religiously check back with this thread with updates.

Hi, I hate to be a downer and I’m not calling anyone a liar but a lot of what I read in these comments seems very hard to believe. I’ve been o. Hormones for two years now and have been researching their effects for over a decade. One thing I can say for sure that I’ve never even heard anyone claim let alone have evidence of is losing height or for that matter hand or foot size. Fort muscle lose to cause such changes you would have to be insanely well muscles to start with, like freakishly so. Also there was a mention of a trimmer tummy in the headline… I wish. The only way you will have a trainer tummy is through exercise, diet and maybe surgery. MTF hrt will make your belly bigger through fat redistribution as the belly is of of the places that your body will start sending more fat, women have to work a lot harder for trim tummy’s and waits lines, that’s a fact. Also someone mentioned have a c cup after a year of hrt, IF this is true it is exceptional and no one should expect to be as lucky. Great devolopment takes years even for cis women and trans women rarely get as much development. A lot of anecdotal evidence says you can expect at best to get a cup size or two less that you’re mother and or sisters if you have any.
The rest is all true from my experience, the skin changes, change in body oder, etc. but don’t expect to just loose all you body hair without help. It does get reduced, grows slower and become softer and all that though.
Sorry if this post offends anyone or basically rains on some parades but I felt that someone should put some more realistic expectations out there.

But just because one trans woman receives the effects doesn’t mean that we all will. Some may get some effects but not others.

I can agree with your comments, I’m 54 years old, 6′, 205, ive always been very athletic so I am fairly muscular, I was on Spiro for 3 years prior, I’ve been on hrt for 1 year, and had both my testicals removed due to a medical problem about 3 months ago so no more Spiro , my voice hasn’t changed, I can still get and maintain an erection depending on my mood-with a thimble full of clear liquid, my hands are still XXLARGE, feet are still size 11-12 in heels, jeans are size 10-12 with my butt is finally starting to get some padding, my boobs are about a B cup or slightly smaller but at least they are rounding out-hard to believe as I’ve never had man boobs only a very well and defined muscular chest so it’s really helped me mentally seeing the change. While I as on Spiro I noticed I never had a sweat smell only until recently after the testical removal and off Spiro my underarm “man smell” has come back with vengeance to the point where I had to start buying deodorant again. I still shave my face and chest every day, but other body hair has showed down considerably, I grew out my hair starting 2 years ago, my nails as permitted by my job and have recently started going to the nail salon for mani & pedicures-what a treat.. I have been married for almost 40 years to the most supportive and loving wife a person could ask for, I was truthful to my wife from the start when I had no idea what was wrong with me and I was scared to mention how I felt, but luckily things have worked out. It’s an amazing feeling when I stopped living my lie and started living who I felt I was, my outlook on life changed for the better, I hope and pray you can all find your happiness no matter what your situation in life has brought you, coming out to my family, friends and relatives has been very positive, all anyone wants is to be happy and live a very long and successful life, it all starts with being who you need to be. I believe the lose in height is due to lose of muscle in the spine which allows for more compression,just like getting older. As for the feet they can also shrink as well. It really depends on the starting point in terms of muscularity.

Hello, and thank you to everyone above who has shared and been honest. I have been on hormones for eight months. I am 28 years old and a little info about my progress is: BEFORE – 5’4″, 150 lbs (very little body fat), acne, crazy amount of body hair, size 10 shoe (womens), male appearance hands, fairly flat chested. AFTER: 5’4′, 125 lbs (hips and butt is noticeably bigger and shoulder mass has gone down a small amount), no acne, very little body hair, size 9.5 shoe (womens) and some brands i can wear a 9, male appearance hands still, and small B cup breasts (had them measured for bra size). I understand the argument with hands and feet and here is what I would deduce…Your feet can get a slight amount smaller for several different reasons. In some Asian cultures woman force their feet to shrink by wrapping them, and it does in fact shrink their feet. HRT also reduces muscle mass and CAN decrease the distance between bones due to the same factor. I have noticed that wearing shoes that have felt tight for quite some time now has impacted the size of my feet. As far as hands go…maybe you can wrap your hands to make them smaller but I am convinced they will not look more feminine unless you somehow learn to burn all of the muscle and fat from your hands and make them look boney. Probably not a healthy thing to do, lol.
I would conclude that HRT will absolutely affect everyone in a different way. Do not assume anything when starting hormones. I never thought I would be a passable female and after 8 months, when someone finds out I am trans they get this shocked look on their face which tells me that it never dawned on them I was anything other than a cis woman. I promise if you take care of yourself, you are responsible, you work hard, and you have faith and patience that you will become the person you have always wanted to. Don’t worry about what hormones are going to do, they will change you physically, just focus on being you.

If you have any questions, feel free to ask me as I will not give you my opinion so much as what my medical professionals have told me. But again, don’t take anyone’s word for it. Yo! Those orgasms though! Omgs listen, I’ve been on hormones since Feb 2017 and my orgasms have gotten super powerful. I remember asking myself is it me or is bustin’ a nut so much better? It’s awesome! But most everything else I can relate to except height or extremity shrinking. Same height and shoe size! I’m short with big feet. Damn 11’s! (43 EU). My boobs are still growing and hurt. My lymph nodes are sore sometimes. My body hair is very fine, except my daggum face, arg! My balls shrunk and I lost an inch on my peenie. Lol but I don’t care. I look more feminine but not like most other people. I wish I would’ve started in my teens instead of my thirties. But HRT is life. When I was rejected for a month bcuz American insurance is TRASH, it fucked me up for several months even after I got back on. My libido went up and my anger was 10 fold during my dark month. Being emotional in the first year was a physical reaction to the hormonal shift. But it’s nothing now. It didn’t solve my gender dysphoria (FFS would solve that by helping me pass) but it’s a great start. All these changes from HRT alone have made my life better. Enjoy your life! I love you all!!

How will adding a hormone to a body that doesn’t support those organs react over time? Will the body end up fighting itself and what happens with that? I feel that you are looking for an excuse to impose your will on your child rather than face the fact that you just don’t want it to happen. However with a couple of exceptions every organ in the male and female body is identical. So much so that a male friend has a kidney from his sister. Both male and female bodies use oestrogen AND testosterone only the levels change. Most of the senior sporting bodies including the Olympics now accept that after 4 years a trans person has similar performance to a person born into that sex. This is proved by the fact that sport isn’t dominated by transwomen. In fact in golf transwomen struggle against born women because of the pelvic bone difference.
The body is constantly fighting itself, every second millions of cells are killed and removed because they have become faulty, they are replaced. Everybody carries the potential for cancer some develop tumours others don’t.
Finally hormone replacement therapy has a huge body of evidence from the millions of post-menopausal women, men with testosterone deficiency, and much else. It is about the most investigated area of medicine there is. Sure any treatment carries a risk of injury or death, I doubt that has been a factor in medication you’ve taken for an illness. Your child could suffer a life of misery and an early squalid death and will certainly be unhappy unless their condition is treated.
If you truly love your child then let medicine help them, if you love them only as long as they conform to your beliefs then you should hang your head in shame.

Well I have nothing against her wanting to do this but I do have an issue with what you call medicine. For your information most medicine are just legal drugs that do more harm than good. So that is more of the problem than my child dealing with wanting to change. And for you to be jumping down my throat for being concerned with my child’s health and well being shows me that you haven’t got a clue of the hell you are talking about and no idea of the true side effects to these drugs.
If I wouldn’t put that shit in my body why would I be ok for my child wanting to do it. You must not have kids because you are clueless on being an actual caring parent who doesn’t want their child to be harmed by some quacks like you.

I understand your apprehension to HRT. It’s an imposing thing. But, under the proper supervision of a good doctor, the risks of HRT are incredibly small. Check out this article: https://bit.ly/3plsl54 .
Realistically, any negative health effects from the hormones would be outweighed by the mental strain of forcing your child to go through the wrong puberty, and the distress that causes.
So, please reconsider your stance on HRT for your child.

Out on 4/18/21, began HRT with once weekly injections through Plume (via phone app, ran by other Trans and Enby folk, $99/mo for all MD support + labs, Meds and supplies separate cost, and NO GATEKEEPING!) on 6/17/21. I have always known something was wrong from as far back as I can remember.My first memory of the incongruity was from age 4ish. I used to go to to my gma’s house often as a kid(I still do actually, I live 3 minutes away from her now), and I’d like to get my nails painted. It was always clear, and one day I asked for red and she told me “No”. Me: “Why not?” Her: “Only girls can get colors”. My gut reaction was to respond “But I’m not a boy, Grammy!” However, lacking a reference level for such things, I nodded and moved on.
FF to middle school: I never really liked other girls, and romance and lust were almost imperceptable. Sex with women was meh,
Eventually Once on HRT tho, turns out I was just green with envy, and my sexuality “flipped” and….OMG HELLO MEN!!!
On 9/11/21, I had my first REAL(organic, not forced/faked)sexual encounter with my best friend since 6th grade, I was definitely not disappointed. (He actually came onto me!)
The creepy thing?? Literally the day prior my dad point blank asked me if he and I had been sleeping together. I responded honestl, “No!” Then less than a day later it happened. Guess there was chemistry we’d been naive of or something…
Also, I recently discovered around Thanksgiving that in addition to the emotions of lust and love, I can now process Grief, and EMPATHY!!! (FYI, My Mom passed of cancer 10 days before my 9th bday in 2001, I was the only one of my immediate family not completely distraught at the funeral, and I knew it. I thought I was a monster for years, a true sociopath. Turns out, I just lacked the hormones to properly experience those things. I didn’t realize it, but I was actually so miserable and angry until I came out and fixed this birth defect, that I was trying to kill myself slowly with alcohol, cigarettes, and excess food and I got up to 284lbs(I prefer birth anomaly tho, cuz I now am full of pure Joy)
I’m a Jane of all trades, I have previously worked as an electrician, auto mechanic, repaired sewage lines, and I just left my last job as a security officer. Today, I’ve lost 130lbs with keto, intermittent fasting, regular exercise, and I weigh ~150lbs at fast. I (mostly) pass w/o makeup, lost about 2 inches in height, went down 2 shoe sizes, now sz 10 dress size, I (mostly) have the voice down(thanks 6 years of choir, and Bria on YT), I found G_d, and I now work with the elderly. Life is finally going well. If you feel like this is who you are, I strongly urge you to look into it. Good luck y’all!
